Just follow both steps below

SKY HD TO SKY+

Press TV button
Hold i + Select until LED flashes twice
Press the number button 1  (2 to revert back)
then press Select
then press Sky

SKY PLUS TO SKY

now press TV button
Hold Yellow + Select until LED flashes twice
Press the number button 0  (1 to revert back)
then press Select
then press Sky

SKY NORMAL TO SKY PLUS

Press TV button
Hold blue + Select until LED flashes twice
Press the number button 2 (0 to revert back)
then press Select
then press Sky

SKY+

 To access RF Menu
Services 4 0 1 Select

SKY+ HD

 To access RF Menu
Services 0 0 1 Select
